A detailed CV is necessary for conducting a Fair Market Value analysis, which appraises an individual’s expertise, qualifications, industry
knowledge, and experience to ensure adequate compensation for services provided. Please see CV checklist below.
CV Checklist:
- Details of trials/research projects (study title and year) in the past 7 years and position held (principal investigator/sub-investigator/consultant)
- Details of presentations given (research/study title, conference name and year) at national, international, and regional conferences in the past 7 years
- Details of time served on committees determining national/international guidelines
- Details of time served as government advisor or advisor to other national or international organizations
- Details of current institutional leadership role (Chair, Co-chair, President, Board Member, Vice President) for therapeutic area and/or specialty
- Details of current and past leadership role (President, Vice President, Secretary, Treasurer, Board of Directors, etc.) of medical association (including patient organization)
- Details of reviewer and editorial board member activities for a peer reviewed medical journal for a therapeutic area and/ or specialty (role, journal name and year)
- Details of national or international scientific awards received
W-9 (US) or W-8 (Ex-US)
The W-9 form is used in the United States for domestic taxpayers. This documentation is used internally by our finance department to facilitate accurate vendor identification, comply with reporting requirements, maintain legal compliance, streamline payment processes, and support thorough audit trails.
It plays a crucial role in ensuring transparency and adherence to tax regulations. The W-8 form is used for non-U.S. taxpayers who are receiving income from U.S. sources. Both forms are part of this package – please submit the form that applies to you.
